Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Multiple containers of food in the walk-in cooler area stored open with no covering. Container of raw chicken not covered, stored directly on the floor; corrected on site. Observed multiple foods being cooled in large stock pots and large tubs in walk-in cooler, which is not a proper cooling method. Refrigerated, ready-to-eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours in the walk-in cooler not being date-marked. Observed multiple deeply scored cutting boards not resurfaced or discarded as required. Interior of the black microwave has old, dried food debris. The microwave is not in working condition and needs replaced. Observed the following food contact surfaces not clean to sight or touch: six-10 knives on a magnetic holder; two peelers; can opener; two hand sinks and a prep sink with buildup of food debris and grease; multiple pieces of equipment with old, dried food on exterior of the units; bain-marie with dried old food debris on interior and gaskets with buildup of organic matter. Condenser unit in walk-in refrigerator is dropping small ice particles on the floor; manager called at time of inspection for emergency repair. Observed extremely low grout harboring organic matter in need of repair. Gaps observed around exterior walls. Management stated floor repair was to begin on Aug. 12. Ceiling tiles missing in the kitchen area and need replaced. Observed several dead roaches on glue traps and two found on the floor in service area. Observed two live roaches in service area.

Person in charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Observed under the cooking suite a glue trap with one dead roach. Increased pest control may be needed.

Food employee observed donning single-use gloves without a prior hand wash. Observed wet-wiping cloths in prep area not being stored in sanitizer solution. Commercially processed refrigerated, ready-to-eat, time/temperature control for safety food in the walk-in cooler held more than 24 hours and is not being date-marked. The prep area is in need of a deep clean evident by dried food residue and paper trash. Exit door in the back storage area has a gap and does not protect against the entry of insects, rodents, and other animals.

Employee observed changing tasks that may have contaminated hands without a proper hand-wash in-between. A food employee was observed touching food with bare hands. Multiple foods in the walk-in cooler stored open with no covering. Observed two raw hamburger packages stored directly over ready-to-eat foods in the walk-in cooler. Observed in-use knives stored between table edges or between tables, an area not easily cleanable and sanitized. Observed wet-wiping cloths in kitchen area not being stored in sanitizer solution. Observed wiping cloths being used for multiple tasks, wiping food spills, working with raw animal foods, etc. Commercially processed, refrigerated, ready-to-eat, time/temperature control for safety food in the walk-in cooler held more than 24 hours and is not being date-marked. Food employees observed preparing vegetables, not wearing proper hair restraints, such as nets, hats, or beard covers; corrected on site. Numerous food storage containers observed throughout the facility not labeled with the common name of the food. Observed noodles cooling at room temperature on a prep table, which is not a proper cooling method; corrected on site. Numerous refrigerated ready-to-eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours in the walk-in cooler is not being date-marked. Mechanical warewashing equipment does not have a manufacturer data plate with operation specifications. Mechanical warewashing machine had buildup of food and moldlike substance on interior and exterior of the unit in need of deep cleaning. Three-bay sink had a large amount of greasy film on the interior of all three bays in need of deep cleaning. Observed the following food contact surfaces all in need of cleaning: grinder/parts with old, dried food debris; large white tub containing wares with food debris on small-wares and in the bottom of the tub; cutting boards with food debris and moldlike substance; juice machine with old, dried food debris; sugar cane extractor with large amount of debris on interior and exterior of the unit. Observed the following on nonfood contact surfaces: buildup of brown/black grease on the dishwashing racks; exterior of the large white rice bin has old food buildup; bain-maries are all in need of deep cleaning including gaskets. Observed clean food equipment and/or utensils in kitchen area, stored wet in a manner that does not allow for draining and/or air-drying (wet-nesting). Observed dish racks stored on the floor in kitchen area. Overhead vents and ceiling tiles have a large amount of static dust in need of cleaning a possible contamination of food and equipment. Drains throughout the facility are in need of cleaning, old food debris and moldlike substance was observed.

Container of slicer and mixer parts were not clean to sight and touch; corrected on site. Shelf above prep table to the right of the stove in the prep area has buildup of static dust. Exhaust fans/floor fans in back prep area have a large amount of static dust and can potentially contaminate food and equipment.

Person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this noncompliant inspection. Observed two open coffee cups with coffee inside stored on a prep table in the front sandwich line; cups were moved and placed on a second food contact table in the back prep area; discussed with management and corrected on site. Observed manager and two employees in prep/cook areas not wearing proper hair restraints, such as nets or hats; corrected on site. Sugar in the back prep area stored open with partial covering. Intentional hole was cut into the lid of the sugar container, which could cause contamination. Observed food ingredient storage containers, bottles and shakers not labeled with the common name of the food. Food-dispensing utensil in sugar and salt in the back prep area observed stored in the food and not with handle above the top of the food and the container. Observed commercially processed food (tomato sauce) was only heated to 107 F before being placed in the steam table and not to 135 F for hot-holding as required; discussed with manager and corrected on site. Observed numerous food items (commercially processed and food prepared in the food facility such as lasagna, bacon, sausage, assorted cheeses, chicken) in the walk-in cooler and upright cooling unit in the back prep/dish area being held for more than 24 hours and not dated-marked. Observed temperature-measuring device on the mechanical dish machine is in need of calibration. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Floor drain in front of pizza oven/bain-marie is open and full of food debris and in need of cleaning and a drain cap. Loose rubber door gaskets observed on the walk-in cooling unit and the ice machine door. Observed the latch at the top of the large walk-in freezer door is broken and inoperable; latch must be repair or replaced. Observed a condensation line from a split-system air conditioning unit on the wall above the three bay-sink, draining directly in the sanitizing sink of the the three-bay sink. Food facility is reusing single-serve/single-use souffle cups; discussed with manager and corrected on site. Black moldlike substance observed on the interior of the ice machine and in need of a deep cleaning. Observed two white buckets with utensils stored and a clear plastic bin with lids with crumbs in the containers coming in contact with equipment stored; corrected on site. Observed single-service, single-use articles stored in back stock area directly on the floor and not 6 inches above the floor. Observed single-service, single-use articles stored in the front and back areas of the facility which were not in the original protective package or inverted to protect against contamination; some, not all, were corrected on site. Observed clean food equipment in the back equipment shelving area, stored wet in a manner that does not allow for draining and/or air-drying (wet-nesting). Observed clean food utensils in bar area stored uncovered or not inverted. Floor tile in the back dry stock area is cracked, broken and missing and is not a smooth, easily cleanable surface. The hand-wash sink in the bar area does not have single-use towels, continuous towels, or air-drying device.
